In order to determine if the residents, homeowners and businesses of Iberia Parish will be eligible for Federal Emergency Management Agency (FEMA) assistance known as Individual Assistance we need your help in reporting any damages that you experienced to your home or business. This request is extremely important as we must be able to show FEMA that our Parish sustained a sufficient level of storm related damages in order to qualify for Individual Assistance. These damage reports will not only help you but your friends and neighbors also.

Therefore, I am asking all businesses and residents to report any Hurricane Laura storm related damages as soon as possible by phone (337) 492-5451 or online at Preliminary Damage Survey Form. When reporting any such damages it is important to have the following information available:

1. Name and Address

2. Contact Information (phone and email)

3.' Pictures of any and all damages

4. What damage was sustained to the home or business

5. Whether you own or rent the property

6. Whether or not you have homeowners or

business insurance coverage

Please remember that providing this information to Iberia Parish Government does not constitute an actual claim for FEMA benefits- it only helps our Parish in seeking eligibility for Individual Assistance. If our Parish is determined to be eligible for individual assistance from FEMA as a result of your initial damage reports, we will provide additional information for submitting your damages claim to FEMA.
